The Problem

A well-regarded private general physician ran a single-doctor clinic with one receptionist. The clinic's WhatsApp number was plastered across Google Business, patient prescriptions, and word-of-mouth referrals. On an average day, 70–90 WhatsApp messages came in from new and existing patients.

The receptionist handled both phone calls and WhatsApp simultaneously — typing responses between booking appointments. Internal data showed the median first response time on WhatsApp was 14 minutes, with some enquiries sitting for 40+ minutes. During clinic consultation hours (10am–2pm and 6pm–9pm) the receptionist didn't touch WhatsApp at all.

The System We Built

We implemented a full WhatsApp automation stack on Meta's Cloud API with GPT-4o powering the conversational layer:

  1. Symptom triage tree with GPT-4o: When a new patient messages with symptoms ("I have a bad headache and fever since yesterday"), GPT-4o runs a guided triage flow — asking 3–5 clarifying questions in natural language before classifying urgency (emergency / same-day / routine / follow-up). Emergency cases immediately alert the doctor via SMS and call.
  2. 40+ doctor FAQ instant responses: Common questions ("What are your consultation fees?", "Are you open on Sunday?", "Do you accept X insurance?") get instant, pre-approved responses in English and Tamil, so the receptionist never types the same answer twice.
  3. Auto-booking into Google Calendar: Patients classified as routine can book a slot directly through WhatsApp by picking from available same-day / next-day times. The booking writes to the doctor's Google Calendar and sends a calendar invite to the patient's phone.
  4. Pre-visit instructions: Every booking triggers a pre-visit WhatsApp message with arrival instructions, required documents (old prescriptions, lab reports), and a prepay-via-Razorpay option (optional, 20% discount if paid in advance).
  5. Post-visit nurture sequences: After a consultation, the patient gets an automated week-1 check-in ("Have your symptoms improved since your last visit?"), a medication reminder at 7pm if a prescription was logged, and a month-3 follow-up booking reminder for chronic conditions (diabetes, hypertension, thyroid).

Results After 8 Weeks

What We'd Do Differently

If we rebuilt this, we'd add voice note handling in the triage layer. About 15% of patients send voice notes instead of typed messages (especially elderly patients and those with poor typing skills) — these currently need manual transcription by the receptionist. A Whisper integration on the inbound message hook would auto-transcribe and route them without human intervention. We'd also integrate the post-visit nurture data with the clinic's existing EHR to pull actual diagnosis and medication names into the reminder messages, making them more personalised.
